Consent to the processing of personal data for the purpose of conducting research and surveys

1. This Consent is granted for the processing of personal data both with and without the use of automation tools.

2. Consent is granted for the processing of my personal data, which are not special or biometric data: surname, first name, patronymic, email address, telephone number, as well as the name of the organization (if such data are provided by me).

3. The purpose of personal data processing: conducting statistical and marketing research.

4. The following actions will be carried out with personal data during processing: collection; recording; systematization; accumulation; storage; clarification (updating, modification); retrieval; use; blocking; deletion; destruction.

5. The period of processing: during the survey/research and within 1 month after the completion of survey/research data processing, but in any case no longer than 1 year from the date the personal data are provided.

6. Consent to the processing of personal data may be revoked at any time by submitting an official request to the Operator in the manner prescribed by the Personal Data Processing Policy.

7. Upon withdrawal of consent, personal data shall be destroyed in the ways specified in the Personal Data Processing Policy.

8. Consent to the processing of personal data is voluntary, specific, unambiguous, concrete, informed, and conscious.

9. By providing this consent, I confirm that I:

   • have been informed of the procedure and possible consequences of withdrawing consent.
